Club Sports Medical Clearance

Students participating in Club Sports are REQUIRED to have their medical information on file with the Providence College Student Health Center. Please follow the instructions below to ensure your medical information is up-to-date. Students who have not submitted their information will not be permitted to tryout or participate in Club Sports.
Note – Students need to submit the information only once, before their first year. Returning students do not need to resubmit health records or immunization information.

1. Login to the Student Health Center Patient Portal using your PC user id and password.
2. On the home screen, look for the “Immunizations” tab. (If you are using a mobile phone, press on the “=” button on the top left to show the full menu.)
3. If there is a red dot next to the “Immunizations” tab (see image below), you are missing medical information. Please click on the “Immunizations” tab and follow the instructions to submit your missing information. If you have questions about any of the requirements, please contact Catherine Kelleher, Executive Director of the Student Health Center, at ckellehe@providence.edu.
4. If there is no red dot, you are up-to-date. No further action is necessary.
